Your affidavit should include:
If you need to attach a document to your affidavit (eg your traffic history or criminal history), this document is called an ‘exhibit’. You need to put or attach a certificate of exhibit on the document. A certificate of exhibit is a statement that confirms the document is the true copy of the document referred to in the affidavit. This certificate of exhibit will need to be signed by the Justice of the Peace who witnesses your affidavit.
